Preventive Health & Care Delivery

The National University Health System (NUHS), in collaboration with the National University of Singapore, is bringing care closer to home through a community-based approach that empowers people to take charge of their health.

By placing a dedicated care team directly in the neighbourhood, we aim to better reach those who may need extra health and social support, build trust, and encourage healthier habits. We believe that building strong, genuine relationships with people in the community is key to creating a system that supports better health and well-being for the long run.

By focusing on prevention — through education, screenings, vaccinations and small behavioural nudges — and ensuring people get the right care, at the right time, in the right place, we aim to detect health issues early and support long-term well-being.

Together with the community, we co-design solutions that match what people need and care about most.

Our key goals:

  • Build trust and connection to create a strong community culture.
  • Empower residents to take charge of their health early by taking simple steps to prevent illnesses, like signing up for HealthierSG, going for regular screenings and vaccinations, and making healthy lifestyle choices.
  • Identify and focus on those who need help the most by bringing together both social and healthcare services to better support them.
